A Proven Model to Improve Patient Care
A medical home is a quality improvement approach that promotes a partnership between the patient, the family and the physician care team. It is a physician-directed model, where the physician leads a team of individuals who collectively take responsibility for the ongoing care of patients. Care is coordinated across all aspects of the health care community.
Studies definitively show that care delivered by physicians in a Patient-Centered Medical Home is consistently associated with better outcomes, reduced mortality, fewer preventable hospital admissions for patients with chronic diseases, lower utilization, improved patient compliance with recommended care, and lower Medicare spending.
How Does MDdatacor Support the Patient-Centered Medical Home Model?
The MDinsight® solution improves timeliness of care, provides physicians with care opportunities derived from evidence-based standards, and monitors patient outcomes so that care processes can be continually improved.
MDinsight® supports a patient-centered approach to care through:
- Web-based access to integrated patient data across provider settings, including hospitals, primary care and specialist practices
- Comprehensive data collection from lab, practice management, EMR and registry systems, as well as transcribed notes
- Evidence-based decision support
- Identification of care opportunities for wellness screenings and chronic patients that are out of compliance
- Evidence-based outcomes analysis
- Electronic referrals to specialists and disease management services, including the sharing of relevant patient data
- Point-of-care reminders
- Patient access to a personal health record
- Secure messaging between providers
Share Data Between Existing, Disparate Sources
The MDinsight® system's advanced technology provides a completely interoperable method of sharing data across health plans, physician practices, emergency departments, hospitals, laboratories, and others within a healthcare community.
MDinsight® analyzes claims data as well as clinical data from any system - lab, registry, EMR, clinical or practice management system - regardless of vendor. This vendor-neutral interoperability allows the collection and sharing of information from existing, and often disparate, information systems.
The Most Comprehensive Patient Data Set
Ability to Analyze Unstructured Data
As much as 80% of medical information exists in the form of unstructured free text. MDinsight® is the only solution that has the ability to pull clinical information from unstructured sources such as transcription notes and the free-form text from EMR Progress notes.
No Electronic Systems Needed
Even physicians who are not using transcription, an EMR or other electronic system can still participate in the Patient-Centered Medical Home model; all that’s needed is Internet access. MDinsight® will provide a customized template for data entry on the Web portal. In fact, the MDinsight® system can be used as an electronic medical record for practices that aren’t currently using an EMR system.
Patient Personal Health Record
MDinsight® can incorporate patient self-reported data into the data that is shared across the community, via the patient portal functionality. Additionally, a patient’s personal health record can be pre-populated with information that the MDinsight® system has collected from the various data sources (EMRs, labs, registries, practice management systems, etc.).
More than Just Data Sharing: Clinical Decision Support
The MDinsight® solution doesn’t just allow the sharing of data – it identifies opportunities to improve care. The aggregated claims and clinical data is analyzed against evidence-based guidelines. Physicians and clinicians can view Web-based reports identifying patients in need of a test, or who have had a test but the value was too high. The reports also show which patients are in need of a preventative screening, such as an annual check-up, colonoscopy or mammogram.
Every time a patient sees a physician, fills a prescription, or has a lab test, the information is added to the MDinsight® system, producing a longitudinal, evolving view of patients’ health and yielding health care cost savings and outcomes improvements.
